臨床上右下腹痛常被認為是腸道疾病、泌尿系統、或骨盆腔疾病相關。本個案是一位17歲男性患者,右下腹痛兩天至本院求治,初步診斷為懷疑右側大腸憩室炎,於疼痛控制及抗生素治療2天後,右下腹部疼痛未改善且加劇,即安排腹部顯影電腦斷層,結果發現右下腹部的小腸環內存在約2.7cm線性高密度病變,高度懷疑異物攝入導致迴腸腸環損傷,會診外科醫師施行切除迴腸吻合術,確診為魚骨攝入引發梅克爾憩室炎併穿孔。術後給予抗生素治療、個案右下腹痛及感染情形改善後出院返家。通常右下腹痛症狀表現者,因早期症狀不典型,易與急性闌尾炎及右側大腸憩室炎混淆,無法儘早診斷;若病人經治療後仍持續出現右下腹痛、反彈痛之症狀,且實驗室檢驗值異常,應再次重新執行詳細病史詢問有無異物攝入、腹部理學檢查、實驗室檢驗及安排腹部顯影電腦斷層檢查來診斷並及時手術治療。

英文摘要

Clinically, right lower abdominal pain is often considered to be related to intestinal disease, urinary system, or pelvic disease. A 17-year-old man visited our hospital with the chief complaint of right lower abdominal pain for 2 days, the initial diagnosis was suspected of right diverticular disease. Two days after pain control and antibiotic treatment, the lower right abdominal pain did not improve and exacerbation, that is, abdomen computerized tomography with contrast revealed a linear high-density lesion of about 2.7 cm in the small intestine ring of the right lower abdomen. It was highly suspected that foreign body implantation caused damage to the ileum and intestinal ring. Diagnosed as fish bone ingestion caused Meckel's diverticulum and perforation. By applying antibiotic treatment after the operation, the patient's right lower abdominal pain and infection have been improved and he was discharged. Since the early symptoms of right lower abdominal pain are atypical, they are easily confused with acute appendicitis and right diverticular disease, and cannot be diagnosed as soon as possible. If the patient continues to show symptoms of right lower abdominal pain and rebound pain after treatment, and the abnormal laboratory tests present abnormal, it will be better to re-execute the detailed history taking to ask whether there is foreign body intake, abdominal physical examination, laboratory test, and arrange abdomen computerized tomography with contrast to diagnose and timely surgical treatment.
