勞動部訂定「辦理勞工體格與健康檢查醫療機構認可及管理辦法」，規範辦理勞工體格與健康檢查醫療機構之條件及管理等事項，希望藉由現場查核，提升其勞工健康檢查自主管理之品質。運用職安署訂定「辦理勞工體格與健康檢查認可醫療機構之檢查品質及管理分級訪查作業要點」之訪查評分表，探討我國近年辦理勞工健檢醫療機構執行勞工健康檢查之品質概況；及瞭解有無國際認證機構認證之醫療機構在勞工健檢品質差異之影響。透過參與「認可醫療機構訪查業務講習或訓練資格」之訪查人員進行實地訪查，擷取105及106年共210家醫療機構為本研究對象。訪查評分包括品質管理、醫事人員管理、儀器設備與實驗室管理、檢查報告管理、以及健康管理分級共六大項評分，滿分為100分。本次訪查結果有59.5%的認可醫療機構具備財團法人全國認證基金會(國內之國際認證機構)「醫學實驗室認證」，有31.0%的認可醫療機構取得財團法人全國認證基金會之「健康檢查認證」資格，以廣義估計方程式分析結果顯示具備醫學實驗室認證(β=11.4, p<0.001)，以及同時通過健康檢查認證之醫療機構(β=11.9, p<0.001)顯著提高認可醫療機構之訪查評分。為保障我國勞工健康檢查品質與勞工權益，並提升健檢品質管理及與國際接軌，建議主管機關可考量將醫學實驗室認證及健康檢查之國際認證納為認可醫療機構之評鑑標準。

The Ministry of Labor has provided the “Regulation for the approval and management of medical institutions for labor health examinations” to regulate the conditions and management of designed medical institutions for labor health examination. We expected that the on-site visits would improve the quality of self-management for labor health examinations. Using the score sheet from Occupational Safety and Health Administration to examine the quality of labor health examinations performed by the designed medical institutions in recent years, and to understand whether there were any differences in the designed medical institutions accredited by the Taiwan Accreditation Foundation (TAF) in this research. Through on-site visits by trained interviewers, 210 designed medical institutions were recruited for the study in 2016 to 2017. The interview scores included quality management, medical personnel, equipment and laboratory management, inspection reports, and health grading by the regulation. A total of six major parts were scored, and the total score was 100 points. In this study, there had been 59.5% medical institutions passed the TAF medical laboratory accreditation and 31.0% medical institutions passed the TAF health inspection certification. The medical institutions that had TAF medical laboratory accreditation (β = 11.4, p < 0.001), and both TAF health inspection accreditation (β = 11.9, p < 0.001) showed higher scores compared to non-accredited group. For the quality of labor health examinations, it is recommended that the government could consider the medical laboratory accreditation and health check accreditation as evaluation standards.
