機構照顧員是身障機構的第一線工作人員,因高頻率地執行移位作業,因而經常發生肌肉骨骼傷害及身心負荷過重之問題。本研究目的欲了解機構照顧者之肌肉骨骼疼痛狀態,並透過肌電訊號,探討左右因子、肩背因子、和輪椅定位因子對於移位作業生理負荷之影響。本研究有9位身心障礙機構之機構照顧員參與。評量工具為北歐肌肉骨骼系統問卷調查表和肌電訊號評估儀。實驗作業為雙人抬抱作業。輪椅定位因子為基本、空間傾倒、仰躺和仰傾四種定位。量測肌群為三角肌後側和腰部豎脊肌群。研究結果指出,疼痛發生的部位平均5.44處。疼痛發生部位、疼痛發生頻率、和疼痛對工作的影響,皆以腰部和手腕最常見。生理負荷方面,右側顯著高於左側(p<0.05),背部顯著高於肩部(p<0.05),輪椅定位因子則未發現顯著影響。本研究發現機構照顧員的肌肉骨骼疼痛問題,除了下背部,手腕疼痛亦不容忽視。抬抱作業的左右不對稱影響大,未來需要兩側交替作業。抬抱作業下背部承受負荷遠高於上肢,建議使用搬運輔具協助抬抱作業。

英文摘要

Institutional caregivers are the front-line staff caring for disabled residents. They often experience musculoskeletal injuries and overloading because of the high frequency of lifting tasks. This study aimed to understand the musculoskeletal pain status of caregivers and to explore the effects of left / right factors, shoulder / back factors, and wheelchair positioning factors on the physiological load of lifting tasks by electromyography. A total of 9 institutional caregivers were recruited. The assessment tools were the Nordic Musculoskeletal System Questionnaire and an electromyography instrument. The experimental task was a two-person lift task. The wheelchair positioning factors were basic, reclining, tilt-in-space, and combined reclining / tilt-in-space. The muscle groups of the back part of the deltoid muscle and the lumbar part of the erector spinae muscles were measured. The results indicated that the average reported locations of pain was 5.44. The most commonly- reported locations of pain were the waist and wrist. These locations also had the highest reported frequency of pain occurrence and were associated with the impact of pain on work. Physiological loads were significantly higher on the right side than on the left side (p < 0.05) and on the back than on the shoulders (p < 0.05), and the wheelchair positioning factor had no significant effect (p > 0.05). This study found that attention should be focused on wrist musculoskeletal and lower back pain in institutional caregivers. Caregivers should work alternately on both sides during lifting tasks. The load on the lower back is much higher than that on the upper limbs during a lifting task, and the use of handling aids is recommended.
