病人及家屬因對傷口自我照護不佳，反覆就醫導致醫療資源浪費，經調查2017整年度急診室病人因傷口感染問題三日內重覆返診人數共162人；其中滯留急診或住院觀察佔26人(16%)，於現況分析後發現：護理師對傷口照護認知低；缺乏傷口照護教育訓練；缺乏護理指導工具；缺乏護理指導標準流程與急診環境干擾多。經擬定改善措施包括：舉辦相關在職教育課程、修訂護理指導單張、建立護理指導標準化流程、製作創意傷口石膏模型、護理指導海報及影片。經執行後，護理師對傷口護理認知正確率由75.8%提升至96.7%，傷口護理指導完整率由77.6%提升至97.8%，病人傷口護理指導滿意度由64%提升至92.3%，達成專案目的。

Patients and their family members of emergency room (ER) are not taking good care of their wounds, and repeated ER visits have led to a waste of medical resources. According to the study in 2017, the number of emergency patients who returned to the ER visit within 3 days due to wound infection, among total 162 patients, there were 26 persons (16%) who were stranded in ER or ad-mitted for further observation. After analyzing the current situation, we found that nurses have low awareness of wound care, lack of wound care education and training, lack of nursing guidance tool, as well as lack of nursing guidance standard operation procedure (SOP) and too much interference in the ER. The proposed improvement measures include: holding on-the-job training courses, re-vising the nursing guidance leaflet, establishing nursing guidance SOP, making creative plaster wound models, nursing guidance posters and videos. After implementation, the accuracy rate of the nurses’ awareness of wound care has increased from 75.8% to 96.7%. Wound care nursing guidance completion rate has increased from 77.6% to 97.8%. Patient satisfaction with wound care guidance increased from 64% to 92.3%. The project goal had been achieved.
