中文摘要

本文為探討一位因事故傷害致多重性骨折及腸穿孔患者,歷經面臨再次手術致身心衝擊之護理經驗。於2018/1/17至2018/2/7護理期間,以Gordon十一項健康功能型態為評估工具,運用會談、觀察、直接照護及病歷查證等方式收集資料並加以分析整理,確立個案有急性疼痛、危險性感染及焦慮三項護理問題。護理過程中,筆者在建立良好護病關係後,鼓勵個案及女兒共同參與照護計畫,擬定適切及個別性護理措施,利用轉移注意力技巧、嚼口香糖等方式,協助個案改善換藥過程及腹脹引起的疼痛問題,並用合宜換藥技術以降低手術傷口感染風險,除提供基本生理照護外,於再次手術前,給予音樂介入及提供感覺性訊息使個案了解手術後的情形,將手術前未知的害怕降至最低,進而緩解焦慮感。期望藉由此照護經驗之分享,作為類似個案身心靈層面護理之參考依據,提供更完整之照護。

英文摘要

This article described a nursing care for one patient who encountered multiple fracture and bowel perforation due to a traffic collision. He suffered the impact of staged surgeries. From 17th January to 7th February 2018, all information of this patient was acquired from the interview, observation, daily care, and chart review as well. Moreover, using Gordon 11 Function Health Patterns, there’re three major items– acute pain, critical infection and anxiety in this case. In our experience, a good nurse-patient relationship helped this patient and his daughter to join our individualized care plan as Team resource management. We used the technique of attention, gum chewing shift to reduce wound pain and bloating during the process of dressing change. With a good quality of wound care, the risk of surgical wound infection was lowered. For the preparation of surgeries, the patient was provided with not only basic physiological care but also music therapy and correct information about his surgeries. It helped him to understand a reality of postoperative condition. Furthermore, it also eased his fear and anxiety as well. In conclusion, with this experience, it will provide us a comprehensive nursing care guideline for complicated patients to support their physiological and psychological requirements.
