α-硫辛酸具抑制神經損傷的抗氧化功能，近年為歐美國家糖尿病周邊神經病變處置之一，國內相關應用度低，但詢問度日增。本案例為一位66歲男性糖尿病周邊神經病變患者，下肢疼痛、感覺異常困擾多年，且提出服用的期望。以實證護理5A步驟，針對α-硫辛酸於此處置型問題成效進行驗證。依序為：口服α-硫辛酸能否緩解糖尿病周邊神經病變引起之疼痛不適；搜尋並選定統合分析及臨床試驗各一篇；以CASP評讀實證指出口服α-硫辛酸600毫克/天，對糖尿病患周邊神經疼痛症狀是安全具成效；案主服用α-硫辛酸600毫克/天，並攝取含硫辛酸食物；用藥五週後疼痛視覺類比指數得分自8-9分降至0分，密西根神經病變篩檢分數由5分降至3分。本案例引實證文獻搜尋之最佳證據，探討並運用α-硫辛酸對於糖尿病患者疼痛性神經病變的成效，提供健康照護專業人員做為輔助治療與照護決策參考。

This case report demonstrated how to adopt the method of evidence-based nursing (EBN) to search the best evidence providing clinical making decision on the use of alpha-lipoic acid (ALA) for managing painful diabetic peripheral neuropathy (DPN) in patients. ALA has been used as one of treatments for patients with peripheral neuropathy in west countries recently, shows high antioxidant capacity to inhibit nerve damage. However, there is lack of information related to this subject in Taiwan even highly concerning among patients. This article explored nursing experience with a 66-year-old male suffering from PDN, asked use ALA since suffered lower limbs numbness, feel abnormal, and painful neurological distress impact over a year. As a nurse practitioner, performed clinical care directly, observation, interview, and various assessments. Through conducted systematical searching of research evidence on four electronic libraries. Moreover, critique of research evidence identified eventually, a randomized clinical trial and a meta-analysis respectively. The neurological symptoms of the case were completely subsided after implementing the recommendation of research evidence, 600 mg daily oral ALA for five weeks regimen, and no any adverse event occurred. This case report sharing may provide health professionals with some references to care the similar patients.
