背景和目的:隨著年紀的增長,會導致動作神經元的細胞凋亡、肌纖維數量減少、肌纖維萎縮以及肌肉質量的流失,因而影響老年人的肌力、爆發力以及耐力。爆發力和老年人功能性表現的關聯較大,會影響他們的坐到站能力、平衡、以及行走表現等。高速阻力訓練可以改善老年人的爆發力,因此,本研究目的為探討高速阻力訓練對於老年人功能性表現的影響。方法:本研究搜尋相關電子資料庫,納入2010年至2021年的隨機對照試驗。使用皮卓量表(Physiotherapy Evidence Database)與2011年版牛津實證醫學中心證據等級分析納入文獻的品質。若有兩篇以上文獻使用相同的功能性測試項目,則使用Review Manager Version 5.4軟體進行統合分析。結果:本研究共納入7篇文獻進行統合分析和文獻回顧,統合分析結果顯示,健康老年人經過高速阻力訓練介入後,與控制組相比於30秒坐站測試(平均數差異=2.70,95%信賴區間:1.42~3.98,p=0.0001)、8英尺起走測試(平均數差異=-1.09,95%信賴區間:-1.57~-0.61,p=0.00001)和6分鐘行走測試(平均數差異=118.00,95%信賴區間:52.17~183.83,p=0.0004)有顯著的改善效果,而與傳統阻力訓練相比,只有8英尺起走測試(平均數差異=-0.56,95%信賴區間:-0.88~-0.24,p=0.0006)有顯著的差異。根據系統性文獻回顧可知,高速阻力訓練介入亦能改善最大肌力、直膝垂直跳的表現和生活品質。結論:高速阻力訓練對於健康老年人的功能性表現有顯著的改善效果。

Background and Purpose: Aging is accompanied by cell apoptosis of motor neuron, decline of amount and size of muscle fibers, and loss of muscle mass, which affects the strength, power, and endurance of older adults. The association is higher between power and functional performance in older adults. Power may affect the ability of sit to stand, balance, and walking performance. It has been reported that high velocity resistance training improved power of older adults. The present study aimed to examine the effects of high velocity resistance training on functional performance in older adults. Methods: A systematic search of the randomized controlled trials from 2010 to 2021 was conducted. Some related databases were searched. The quality of eligible studies was evaluated using PEDro Scale and Oxford classification. Meta-analysis was conducted using Review Manager Version 5.4 software. Results: Seven studies were included in meta-analysis and systematic review. The results of meta-analysis showed that high velocity resistance training significantly improved 30-second chair stand test (mean difference = 2.70, 95% confidence interval: 1.42 ~ 3.98, p = 0.0001), 8-foot up-and-go test (mean difference = -1.09, 95% confidence interval: -1.57 ~ -0.61, p = 0.00001), and 6-minute walking test (mean difference = 118.00, 95% confidence interval: 52.17 ~ 183.83, p = 0.0004) comparing to the control group and significantly improved 8-foot up-and-go test (mean difference = -0.56, 95% confidence interval: -0.88 ~ -0.24, p = 0.0006) comparing to traditional resistance training group. According to the results of systematic review, high velocity resistance training also significantly improved one-repetition maximum, countermovement jump, and quality of life. Conclusion: High velocity resistance training showed significant improvement in functional performance among older adults.
