背景:面對高齡社會和子女無法照顧長者的問題,老人大多入住於長照機構,如何留住老人人生的正向經驗,使其活著有存在價值?

目的:本研究目的在敘說由一位長照機構老年婦女的生命故事,進而衍生出十位老人人生的正向經驗。

方法:本研究採敘事研究法,以立意取樣方式,半結構訪談十位符合條件的長照老人,資料收集由2018年12月1日至2019年9月30日,IRB FR -002-1020430 -1-2。資料分析乃依據Lieblich et al. (2008)敘事研究之「整體-內容」分析模式,及遵從其信效度研究嚴謹度準則,於資料分析飽合後即停止收案。

結果:首先,一位老年婦女描述其人生的縱向故事,接著導引其敘說橫向人生故事,研究者再透過人生正向故事捲軸,引導本老年婦女每天看和思想自己的正向人生經驗,使其充滿正能量,以面對自己老化過程的疾病和死亡。延續本老年婦女的成果,研究者再半結構訪談9位長照機構老人故事,並統整其人生正向經驗,及製作其人生正向故事捲軸。

結論/實務應用:老人機構中的長照護理人員於老人身、心、社會、靈性衰退的過程中,可運用其人生故事所衍生的正向經驗,使其順利地過渡至人生的最後階段。

英文摘要

Background: Due to various aging associated problems and the inability of children to care for elders, elders often live in long-term care facilities. Ensuring the elders having a positive experience regarding their existential value is crucial.

Purpose: This study presented a narrative description of the positive experiences of ten elders derived from the life story of an elderly woman in a long-term care facility.

Methods: We used a narrative research method and conducted a semi-structured interview with ten elders recruited from a long term care facility through purposive sampling . Data collection was conducted from December 1, 2018, to September 30, 2019, and the study protocol was approved by an institutional review board (IRB FR-002-1020430-1-2). Data were analyzed according to the analytical model of narrative processes proposed by Lieblich et al. (2008) following the rigor principles of reliability and validity until data saturation was reached, after which data collection was discontinued.

Results: Initially, the elderly woman described her longitudinal life stories, and the researcher guided her to narrate her horizontal life stories. The researcher encouraged the elderly woman to reflect on her life’s positive experiences daily, so that she could experience the positive energy required to face her illness and impending death during her old age. Building on the results of the elderly woman, the researcher conducted semi-structured interviews with nine elders in the long-term care facility and produced positive life story scrolls by integrating their positive experiences.

Conclusion/Clinical Implication: Long-term care nurses in elderly care facilities could apply the positive experiences derived from the elders’ life stories to help them maintain their physical, mental, social, and spiritual well-being during their aging process to ensure a smooth transition through their final stage of life.
