本文係探討一位直腸癌術後合併小腸阻塞76歲男性個案之照護經驗，照護期間於2021年4月21日至4月28日運用觀察、訪談、傾聽、身體評估、直接照護、收集病史及周全性老年評估進行整體性評估，並確立個案主要健康問題依序有：（1）活動無耐力與有術後肺擴張不全有關；（2）無效性因應能力與自我照顧能力降低有關；（3）潛在危險性出血與腹部術後傷口有關。在護理過程中與個案及家屬建立護病關係，藉由教導噘嘴式呼吸、深呼吸訓練器使用、鼓勵下床活動、與呼吸治療師及復健師共同協助個案，每日練習深呼吸訓練次數及增強下肢肌力運動，增進個案肺擴張能力以提升術後活動耐力，盡快使體力恢復到術前狀態；鼓勵個案說出心中感受，並教導家屬讓病人漸進式增加自我照顧的程度以改善個案的因應能力；教導腹部術後傷口照護及使用抗凝血劑注意事項，以預防不必要的損傷及出血。期望藉由此護理經驗，提供臨床照護直腸癌術後合併腸阻塞及高齡者術後照護之參考依據，以提升其照護及生活品質。
