目的:本研究乃針對特殊勞工巡迴體檢時噪音作業之以聽力車執行的聽力檢查結果判定為三級聽力管理為目標族群,逕行初步的變項資料分析與探討。

方法:收集於2017-2019年到本院門診之噪音作業管理三級複查綜合判定之勞工為目標族群,採用橫斷式研究設計來探討影響管理分級的因子。共有666名勞工被納入統計分析。統計方法為獨立樣本t檢定、卡方(獨立性)檢定來、二項式羅吉斯迴歸模型分析,並採p<0.05為統計顯著性的最低判定標準。

結果:409人(61.4%)被認定為職業性聽力損失之第四級管理,其餘257人(38.6%)的聽力損失被歸因於非職業性原因(即屬第二級管理)。518人(77.8%)的年齡在40歲(含)以上(總體平均年齡約46.5歲)。40歲(含)以上的勞工有顯著較高之機率(OR=2.00,p<0.01)被重新判定為第四級管理者。台灣男性勞工也有顯著較高之機率(OR=2.35,p<0.05)被重新判定為第四級管理者。

結論:資深男性員工為主,若未有適當的聽力防護計畫與措施,造成職業性聽損的機率高。更需注意的是外籍勞工未來會取代上述族群,他們需有更好的溝通與相關預防措施,才能給予正確的級數管理與選配工建議。

英文摘要

Background: This study is aimed at the target group of the level III hearing management determined by the hearing test which performed at the working area during the specific labor physical check-up. And conducts preliminary variable data analysis and discussion.
Methodology: The workers who were comprehensively judged by the level III management classification of occupational hearing loss (MCOHL) review of hearing ability evaluation in the occupational OPD from 2017 to 2019 were collected as the target group. A cross-sectional research design was used to explore the factors that affect the classification results. A total of 666 workers were included in the statistical analysis. Statistical methods are t-test, chi-square (independence) test, binomial Logit regression model analysis.
Conclusion: 409 people (61.4%) were identified as the level IV MCOHOL, and the level II remaining 257 people (38.6%). 518 people (77.8%) were over 40 years old (inclusive) (the overall average age was about 46.5 years old). Laborers over 40 years old have a significantly higher probability (OR=2.00, p<0.01) to be re-evaluated as level IV. Taiwanese male workers also have a significantly higher probability (OR=2.35, p<0.05) to be re-evaluated as level IV.
Discussion: Mostly senior male employees, if there is no proper hearing protection plan and measures, the probability of occupational hearing loss is high. It is even more important to note that foreign workers will replace the above-mentioned groups in the future. They need better communication and related preventive measures in order to give correct MCOHL.
