本篇為一位急性淋巴性白血病的病童因化學治療後造成發燒、皮膚與口腔破損的護理經驗,護理期間為2019/10/12至2019/10/20,以運用Watson關懷照護的概念來協助病童及其主要照顧者。以整體性評估及觀察、會談及查閱病歷收集資料,確立問題有:營養少於身體需要、潛在危險性感染、知識缺失、親職角色緊張。藉由主動關懷與傾聽,建立信任的關係,個案運用遊戲治療方式,以增進飲食量,再利用視頻與繪本,學習如何正確洗手、戴口罩等,而案母則提供化療後正確照護知識、技能與社會福利補助等,並結合跨領域的專業,使得案母能瞭解白血病疾病治療計劃,以協助提升照護信心,陪伴其調適與面對不確定的預後困境。建議兒科護理師能針對兒童癌症在職教育,加強關懷溝通技巧與臨床實務分享,方能面對兒童癌症之案母焦慮情緒時,更能從容以對。

英文摘要

This article is about the nursing experience of a child with acute lymphoid leukemia who had fever and skin and oral complications after chemotherapy. The nursing period was from October 12, 2019 to October 20, 2019, with the use of Watson's human care theory to help the child and the main caregiver. Through observations , interviews and medical records to collect information, we raised the following nursing issues: nutrition and energy deficiency、 potential risk of infections、 lack of knowledge、 parenting stress. Through active care and listening, the author established a relationship of trust. We used game therapy to increase the amount of oral intake. Videos and picture books were used to educate how to wash hands correctly and wear masks. We also provided the patient's mother the knowledge and skills of post-chemotherapy care, and the information of social welfare and subsidies. By combining with cross-field expertise, we accompanied the patient's mother to face the uncertain prognosis contradiction. Our cross-field team also introduced the treatment plans to the case's mother to improve her care confidence. We recommended on-the-job education of childhood cancer to pediatric nursing staff, to strengthen care communication skills and share experience of clinical practice. With the education, the nursing staff can be more confident to deal with the patient's and their family's anxiety and emotions.
