本文描述一位中年男性因糖尿病併壞死性筋膜炎，導致左下肢截肢之心理調適照護過程。照護期間2018年12月14日至2018年12月29日，筆者透過觀察、會談、查閱病歷等方式收集資料，並以Gordon十一項功能性健康型態評估，確立個案主要健康問題為：急性疼痛、無效性健康維護能力、身體心像紊亂。透過主動關懷、陪伴及同理心建立良好護病關係，藉由個別性護理措施緩解術後引發的疼痛；運用護理專業知識及照會營養師，教導個案糖尿病照護知識及胰島素注射技能，來提升個案自我照護行為；對於個案因截肢造成的心理衝擊、對未來的不確定和憂鬱之負面情緒，以同理心陪伴病人，慢慢引導個案正視身體外觀的改變，再偕同物理治療師幫助個案盡早恢復日常生活活動功能，強化家庭支持系統增強信心與希望，來陪伴個案度過生理、心理及社會之衝擊，而能以正向態度面對往後的生活。希望藉此臨床經驗分享，作為日後護理人員照護此類病人參考。

This article describes the nursing care of the psychological adjustment of a middle-aged man who left lower extremity amputation due to diabetes and necrotizing fasciitis. Nuring care was provided from December 14, 2018 to December 29, 2018. Gordon's 11 functional health pattern assessment as well as observation, interviews, and reviewed medical records etc were used to collect the relevant data. The patient's nursing problems were acute pain, ineffective health maintenance, and body image disturbance. The author established a good relationship between nurses and patients through active care, companionship and empathy, and relieved postoperative pain through individual nursing measures; used professional nursing knowledge and consulted nutritionists to teach patients about diabetes care and insulin injection skills to improve Individual Self-Care Behavior. For the psychological impact of amputation, future uncertainty and negative emotions of depression, accompany the patient with empathy, slowly guide the patient to face the changes in appearance, and then work with the physical therapist to help the patient return to activities of daily living as soon as possible, strengthen family support system, enhance confidence and hope, accompany patients through physical, psychological and social shocks, and face future life with a positive attitude. It is hoped that this clinical experience will be shared as a reference for nursing staff to care for such patients in the future.
