本文描述一位58男性因工作不慎受傷導致右手前臂被機器捲入，因傷口癒合不佳導致右手前臂截肢，於2020年6月18日到6月22日藉由直接照護、觀察、會談、傾聽、病歷查閱等方式收集資料，以Gordon十一項健康功能型態，進行評估發現個案有急性疼痛、組織完整性受損及無望感等健康問題；並於換藥過程隨時觀察個案非語言反應及感受；持續密切監測生命徵象、皮瓣及其他術後傷口，以避免傷口感染徵象，讓個案共同參與及了解傷口及皮瓣照護；建立良好信任護病關係，給予陪伴與關懷，觀察個案非語言及心理感受，鼓勵說出對創傷後面臨截肢的想法，利用個案平日生活祭祀儀式習慣，藉由宗教信仰力量，以靈性照護增加個案安全感及正向面對未來生活，讓個案願意主動參與自我照顧意願，使其能面對身體外觀上的改變重新適應生活。

This article described a 58-year-old male who injured his right forearm by accidentally rolling in a machine during work. The right forearm was amputated and was reconstructed with a free flap due to poor wound healing. We collected data through total care, observation, meeting, hearkening, and medical record review from June 18th to June 22nd, 2020. The case was assessed using Gordon 11 Function Health Patterns and was found to have health problems including acute pain, impaired tissue integrity, and hopelessness. During wound caring, we continuously observed the case's nonverbal reactions and feelings, vital signs, flap, and post-operation wound to avoid wound infection. The case was encouraged to participate in wound and flap caring to establish a good nurse-patient relationship. We also encouraged the case to express thoughts about traumatic amputation through accompany, nursing care, observation of nonverbal and psychological feelings. The spiritual care, including the sacrifice ceremony and the religions belief was applied to increase the case's security and positive sense to face future life. The above-mentioned nursing care strategies could help the case increase the willingness to actively participate in self-care and adapt the life due to changes in physical appearance.
