這是一位中年男性病患，一個月前剛經歷一年來的第二次突發性暈厥被送住院，經醫院檢查診斷爲病竇綜合徵，建議裝置永久性節律器。出院之後病患希望先嚐試以藥物控制病情，不過一個月來其心悸頭暈仍然天天發作，故求助於中醫。來診時病忠主訴每日陣發心悸、心跳時快時慢已一個月。伴隨疲倦、頭暈、嗜睡。證見面色蒼白、身材偏瘦，平時畏寒，冬天手足端易冰冷，上坡容易氣喘、遇風冷時常鼻塞，伴隨流清涕。問之其實已有十年之心律不整病史。診之脈濡細無力，兩尺脈弱。辨證爲心腎陽虛，氣血虧虛。處方先以養心湯合八味地黃九，養心溫腎控制心律不整，而後加入滋陰養血藥物以鞏固療效。病患目前仍於中醫門診追蹤，心律不整未再發作，病情初步獲得控制。

Attributed to sudden syncope second time a month ago, the middle-aged male was admitted to a teaching hospital, where sick sinus syndrome was diagnosed, and placing a permanent pacemaker was recommended. After discharged, taking medication for controlling was first choice. Otherwise, palpitation and dizziness occurred daily, thus he came to Chinese medical department for help. On the first visit, his chief complaints were paroxysmal palpitations and irregular heartbeat, both had occueed every day for one month, with weariness-, dizziness-, and lethargy-accompanied. The symptoms were the: pale complexion; thin figure; cold intolerance frequently; cold extremities often in winter; tendency to pant after going up a slope; cold air-stimulated stuffy nose making clear mucus. He remarked 10-year history of arrhythmia. The pulse was felt: fine; forceless; soggy; weak at both cubit positions. The holistic differential diagnosis was Heart and Kidney Yang deficiency, combined with Qi and Blood deficiency. The prescriptions are Heart-Nourishing Decoction with Rehmannia Eight Formula for heart strengthening and kidney warming to control cardiac arrhythmia. Afterward, tonifying yin and enriching blood were to consolidate the curative effect. The patient has been followed up at our outpatient department, no arrhythmia recurred up to now, the condition was curbed preliminarily.
