腎周圍膿腫形成的原因大多為血液感染或尿路阻塞引致上行性尿路感染，而由腎內膿腫破潰至腎周圍組織。傳統上，對於腎周圍膿腫以外科開刀引流或經皮穿刺引流法治療，但因尿路阻塞存在的病因，常需作第二階段輸尿管的處理。馬偕醫院台東分院於民國79年3月經歷一位因子宮頸癌、尿路阻塞而併發右側腎周圍膿腫病患。由電腦斷層掃描，明顯可見破潰腎皮質，使腎集尿系統與腎周圍組織相通，我們嘗試利用內引流法一次解決膿瘍引流與尿路阻塞的問題，療效良好。茲提出與大家討論之。

We report a case which demonstrates that internal drainage with double J catheter is considered as an alternative to surgical intervention in the treatment of per-inephric abscess, especially when there is persistent ureteral obstruction and communi-cating with collecting system.
