背景:病童接受心臟手術治療術後,所裝置的許多醫療儀器設備及母親對心臟手術相關知識的不足,常使母親面臨龐大的焦慮。因此手術前的護理指導方式及如何有效提供母親有關病童心臟手術的護理指導為兒童加護護理工作中重要議題。

目的:比較兒童心臟手術前護理指導影片與常規衛教對病童母親焦慮與兒童心臟手術相關知識之影響與差異。

方法:本研究採類實驗研究設計,針對某醫學中心兒童加護病房,進行心臟手術病童母親共60 位收案,先收集對照組30位實施常規衛教,再收集實驗組30位接受護理指導影片。兩組母 親於護理指導前、後進行問卷測量。以「母親及病童基本資料表」、「貝氏焦慮量表」及「兒童心臟手術相關知識問卷」為測量工具,資料以SPSS18.0 for Windows 進行描述性、卡方及獨立t檢定等統計分析。

結果:兩組母親在接受護理指導前均呈現焦慮情形,於接受不同方式的護理指導後,母親對病童 心臟手術相關知識提升及焦慮降低均具有成效,但實驗組母親在知識提升及焦慮降低效果 優於對照組並達顯著差異(p< .001)。

結論:兒童心臟手術前以影片方式提供母親護理指導,能有效提升手術相關知識並降低焦慮,值得兒科相關單位應用及推廣。

英文摘要

Background: Mothers of children receiving cardiac surgeries usually face enormous stress due to lack of care knowledge and the need to stay in an unfamiliar hospital environment. Providing pre-operative instructions for these mothers is very important for nurses in hospital pediatric intensive care unit.

Purpose: The purpose of this study was to evaluate the effectiveness of a video-based nursing instructions in increasing the care knowledge and decreasing the anxiety level of mothers whose child was admitted for a cardiac surgery.

Methods: This study used quasi-experimental design to examine the level of mothers’ anxiety and knowledge in a pediatric intensive care unit in a medical center. A total of 6o participants whose children were hospitalized for cardiac surgery joined this study. Thirty mothers were assigned to the control group that received routine nursing instructions, while the other 30 were in the experimental group that received video-based instructions. Before and after the study intervention, all mothers received a test of Beck anxiety self-rating scale and a questionnaire about knowledge related to the surgery and care. Data was analyzed by the SPSS 18.0 for Windows using descriptive statistics, chi-square, and independent t test.

Results: All mothers showed an improvement of their anxiety and knowledge. Mothers who received a video-based nursing instruction showed a significant better improvement of anxiety and knowledge.

Conclusions: This study concluded that the video-based nursing instruction program was effective in decreasing mother’s anxiety level in comparison with the routine one. It is recommended that the videobased instruction can be applied in the pediatric intensive unit for mothers whose child is receiving a cardiac surgery.
