洗手是預防院內感染中最簡單也是最有效的措施之一，但北部某區域醫院內科醫護人員的洗手遵從性偏低，僅16.2%。故成立跨單位團隊提升醫護人員洗手之遵從性。經現況分析後發現醫護人員洗手遵從性偏低的原因包括：洗手之認知不足、洗手設備不足、宣導不足、缺乏獎勵辦法，因此運用PRECEDE Model 改變醫護人員的洗手行為，採取宣導、張貼洗手提醒標誌和海報、教育訓練、修訂洗手設備補充規範、設置獎勵辦法和回饋單位洗手遵從性之後，醫護人員的洗手遵從性提升至61.2%，達專案目標。本專案之實施不僅改變醫護人員的洗手遵從性，也提供病患一個安全的醫療就醫環境。

Handwashing is one of the very simple and effective methods to prevent nosocomial infection. However, the handwashing compliance among health care providers was very low (16.2%) in medical units. Therefore, the purpose of this project was to improve handwashing compliance in health care providers. According to our observation and analysis, we found that the causes related to low handwashing compliance were identified as inadequate knowledge about hand hygiene, deficient washing equipments, deficient promotion policy, and lack of financial rewards. Thus, the PRECEDE Model was applied to this project to change health care providers' handwashing behaviors. After implementing multiple intervention strategies, including promotion, education, posted symbols and posts, revised supplemental standard of handwashing equipments, and provided handwashing compliance feedback to the units, set financial rewards. The rate of handwashing compliance in health care providers increased to 61.2%, achieve the aim of this project. The project was not only to improve the handwashing compliance in health care providers, but also to provide the safety of health care environment.
