本文主旨在運用團體治療活動照護一位8 歲之過動症兒童,改善其暴力行為、情緒調適 與社交互動障礙的護理經驗。筆者以「整體性護理評估」為基礎,藉由會談、活動觀察、門 診追蹤、病歷參閱及直接照護等方式收集資料。確立個案主要健康問題,包含有高危險性暴 力∕與衝動控制力差;情緒調適障礙∕與不適當個人壓力因應;社交互動障礙∕與溝通技巧 缺失。經建立治療性人際關係,以團體治療活動策略蘊含於護理過程中,每週一次,每次50 分鐘共三次活動。護理過程中,以同理心、接納與支持的態度,鼓勵個案表達情緒,維持治 療方案一致性與執行效能,並提供家屬情緒支持與疾病護理知識,增加個案自我控制能力、 進而提升生活自理能力,促進其適應與健康生活。期望藉此護理經驗分享,能提供護理師照 顧類似個案時,運用團體治療活動策略改善個案問題行為之參考,進而提升兒童精神疾病患 者與家庭照顧者之照護品質與福祉。

英文摘要

The purpose of this case study described a nursing experience witch applied the group therapy activity to take care of an eight-year-old child with attention deficit hyperactive disorder. The caring period of time was from July 1 to July 23th, 2013. We have collected the information and made the holistic nursing assessment through the way of interview, activity observation, outpatient follow and the patient’s medical record and nursing care to collect data. The primary health problems were identified including high risk of violence, emotional adjustment disorders, and impaired social interaction problems. Also, establishing therapeutic relationships. The Nursing process was embedded with group therapy activity involving once a week for 50 minutes a total of three times. We used empathy, acceptance and supportive attitude, and also encourage cases to express emotions, to maintain consistency with the treatment program execution performance, and provide emotional support for family members with the disease nursing knowledge. Furthermore, increasing cases of self-control, and thus enhance the daily living to promote their adaptation and healthy life. We suggest the nurses can provide appropriate nursing intervention by applying group therapy activity to improve the treatment of active cases of problem behavior, and thus enhance children's mental illness and the family caregiver quality of care and well-being.
