本單位經調查發現早產兒出院前準備完整率及滿意度低,分析原因為護理人員臨床工作忙 碌,衛教進度未落實交班、照顧者工作忙碌,無法配合衛教時間,錯失學習機會、照顧者學習時 間短及實際操作次數少,學習成效不佳、衛教本專業術語及文字過多、字型太小,衛教成效不彰、 病室缺乏獨立空間提供仿居家出院前準備。小組介入:與主要照顧者共同擬定出院照護計畫、在 「出院指導衛教本」中加入照片及圖片、陪嬰時間或次數依家屬需求增加、提供仿居家軟性環境 營造病室居家氛圍及使用專案小組設計的「畢業證書」落實出院衛教指導交班等方案後,護理人 員執行早產兒出院衛教完整率由70% 提升至95%,早產兒家屬對出院前準備滿意度由80.1% 提升 至89.2%。早產兒照護團隊運用仿居家概念措施確實有利於早產兒出院前準備,以達早產兒照護 之連續性,強化以家庭為中心的照護。

英文摘要

The survey in our intermediate care unit found that the completion and satisfaction rates of preparation prior to discharge in premature infants were low. After analyzing, the reasons for such low responses included heavy clinical duties to be completed by nurses, housework of caregivers hindered them to join the education schedules, limited time periods and times for caregivers to accompany their infants, small fonts in the educational booklets, excessive use of professional jargons in the educational materials, and insufficient independent space that is similar to home environment. In order to provide a better premature infant care, we rearranged time schedule of learning program of premature infant care to make it available for participants, added photos or graphs in the discharge guidance brochure, and adjusted infant accompany system, provided a ward that is furnished home-like environment, designed a diploma program to ensure shifts are completed. After the implementation of these measures, the integrity of the process increase from 70% to 95%. The satisfaction rate from the families of the premature infants increased from 80.1% to 89.2%. The concept of home care team of premature infants is to establish a home-like environment caring system that is indeed beneficial to premature infants prior discharge and to achieve the continuity care of the premature infants and strengthen the concept of family-centered care.
