The study focused on investigating the biphasic to-and-fro motion of cerebrospinal fluid (CSF) through the aqueduct of Sylvius, largely synchronized with cardiac pulsations. The primary objective was to quantify through-plane aqueductal hydrodynamics by comparing the flow void scoring method with the cine phase-contrast (PC) technique in magnetic resonance (MR) images, specifically in different pathological conditions such as normal volunteers, communicating hydrocephalus (CH), and normal pressure hydrocephalus (NPH) patients. For the assessment of flow void scores, we adopted MR pulse sequences, namely Spin Echo (PDWI & T2WI, TE/TR=20,120/2860ms) and Turbo Spin Echo (T2WI, TE/TR=99/4000ms). Achieving higher spatial and temporal resolution in through-plane imaging was crucial for precision and accuracy. The cine PC imaging technique (in 256x256 matrix, FOV=10cm, and retrospective gating) was utilized. The aqueductal images were segmented using the modified pulsatile-based segmentation (PUBS) algorithm, and parameters were calculated from the velocity-time curve. Subsequently, correlations were established between the flow void score and the pertinent parameters derived from PC images. This project has provided valuable insights into the hemodynamic of the aqueduct, offering evidence and suggesting the potential necessity for surgical shunting operations in patients with NPH and CH.
