背景和目的:瞭解急診護理人員病人安全文化及其影響因子,並進一步與全院資料比較,探討急診與全院護理人員病人安全態度的差異。方法:採用安全態度量表(Safety Attitudes Questionnaire, SAQ) ,涵蓋「團隊合作」、「單位安全風氣」、「對管理的感受」、「工作狀況」、及「對工作的滿意」五個構面,瞭解急診護理人員在問卷各安全態度構面的分數,比較不同人口學特徵護理人員各構面得分的差異;並與全院性資料比較,探討各構面分數的影響因子。結果:全院共有356 位護理人員填答(回收率為68.1%) ;急診共有32 位護理人員受訪,問卷回收率為55.2%,SAQ 問卷內部一致性Cronbach's alpha 為0.62~0.87 。參與者在回答「團隊合作」上得到最高的分數(71.9 ±11.5) 。「團隊合作」與「安全風氣」以及「對管理的感受」與「對工作的滿意」都有明顯的相關。與全院的資料比較下,急診護理人員病人安全問卷調查在各構面得分皆顯著較低(p <0.05) 。結論:應用SAQ 問卷進行病人安全文化調查,可瞭解急診護理人員對安全文化的認知,未來可搭配品質管理手法、病安教育的介入,縱向追蹤急診安全文化與病人安全提昇之間的聯繫性。

Background and purpose: Using a structured questionnaire, the objectives of this study were to investigate patient safety attitudes among emergency department (ED) nursing staff and to compare their safety attitudes with those of nurses from the entire hospital. Factors that may have influenced ED nurses' patient safety attitudes were explored. Methods: The Safety Attitudes Questionnaire (SAQ) was used to assess 5 dimensions of patient safety attitudes from the ED nursing staff. The patient safety dimensions included: teamwork climate, safety climate, perception of management, working conditions, and job satisfaction. Dimension scores were compared among age, gender, and career experience groups. Multivariate regression models were used to investigate the factors affecting patient safety attitudes. Results: Theentirehospital patient safety attitude data were obtained from 356 nurses (with a return rate of 68.1%). There were 32 ED participants with a response rate of 55.2%. Cronbach's alpha of the SAQ ranged 0.62~0.87. ED participants had the highest score for the teamwork climate dimension (71.9 ±11.5). There was a significant association between teamwork climate and safety climate after adjusting for age, gender, and career experience using the multiple-regression model. Perception of management was also significantly associated with job satisfaction. The ED nursing staff's patient safety attitudes were inferior to those of nurses from the entire hospital, as can be seen from the lower scores for all 5 SAQ dimensions (p < 0.05). Conclusions: The SAQ survey data provide baseline patient safety attitude information on nursing staff in the ED. The data can be used to follow-up on the effectiveness of quality improvement campaigns and patient safety education in the future.
