背景:臨床照護上常見長期臥床的病患因尿道感染或皮膚破損導致感染而反覆入院,失去排尿功能的個案其導尿管的使用為維持基本排尿需求。如何協助個案適當且安全的固定導尿管,避免皮膚受到管路壓迫、管路拉扯、或膠帶固定所造成的皮膚損傷等,是臨床照護需加以關注的議題。研究目的:了解導尿管紙膠固定之照護現況。研究方法:本研究以立意取樣選取,第一階段質性研究,針對20位導尿管使用之病患進行訪談;第二階段針對導尿管紙膠固定法進行滿意度調查。研究結果:119位個案及照顧者填寫結構式量性問卷,104份有效問卷分析中顯示,顯示超過一半的個案出現皮膚出現癢的現象(n=56,53.8%)、有異物(n=59,56.7%)、皮膚出現乾燥,不易黏貼紙膠(n=61,58.7%)、皮膚出現紅疹現象(n=70,67.3%)、皮膚出現破皮現象(n=70,67.3%)、黏貼之紙膠會出現潮濕現象(n=71,68.3%)及皮膚出現水泡現象(n=75,72.1%)。另外,超過六成以上的個案表示很少出現移除紙膠後,皮膚會有膠質殘留(n=63,60.6%)、黏貼部位之紙膠會出現脫落現象(n=65,62.5%)、紙膠會殘留在尿管管路上(n=69,66.3%)及黏貼之紙膠會出現變形現象(n=69,66.3%)等問題。進行導尿管紙膠固定法之滿意度分析,顯示皮膚出現壓痕現象(r=-.209,P=.033)、發紅現象(r=-.202,P=.040)、有異物(r=-.346,P<.0001)、移除時皮膚會有疼痛感(r=-.296,P=.002)、移除紙膠後,皮膚會有膠質殘留(r=-.259,P=.008)及黏貼之紙膠會出現變形現象(r=-.201,P=.040)具相關性。進一步分析,影響導尿管紙膠固定法之問題的重要預測因子分析,呈現導尿管固定方式的不同與皮膚出現壓痕現象(r=-.226,P=.021)及皮膚會有膠質殘留現象有相關性(r=-.289,P=.003)。結論:本研究之結果值得提供長期照護單位之臨床護理人員作為護理專案之主題依據,期待護理人員能研發產品來取代紙膠固定法,及設計之原理須能呼應研究之結果建議,包括增加病患舒適感、降低感染及皮膚損傷的發生率。

英文摘要

Background: A urinary catheter is a tube placed in the body to drain and collect urine from the bladder, is the most important way to maintain urination function for urine incontinence patient. Appropriate and proper fixed catheter can avoid skin problem and tubing dislocation. However, tape fixed may cause skin problem, such as itch, redness, rash, tubing linking, and remove with laceration or soreness. Hence, it is need to be concerned about how to reduce the complication by tape fixed. Purpose: Applying the structured questionnaire to understand the satisfaction of Urinary Catheter Fixation in Clinical Setting. Methods, firstly, 20 patients had involved in the quality research, hence three theme were made to build up the structured questionnaires. Result: Total of 119 case filled up the structured questionnaire, 104 were in data analysis, hence, more than half of cases had reported that skin problem of itching (n = 56,53.8%), feel uncomfortable (n = 59, 56.7%), too dry to fixed 3M (n = 61, 58.7%), red rash (n = 70, 67.3%), lesion (n = 70, 67.3%), 3M with wet (n = 71,68.3%) and blister (n = 75,72.1%). However, seldom of them have the problem of gummy residue (n = 63,60.6%), dislocation (n = 65,62.5%), and deformation (n = 69,66.3%). Finally, to analysis the important predictor of the effeteness of traditional catheter Fixation Strap (TCFS), the result shows that the way of fixed was the main predictor, as the relationship with surface indentation (r = -.226, P = .021) and gummy residue of skin (r =-.289, P = .003), indeed, in the satisfaction of the traditional catheter Fixation Strap (TCFS), it showed that the lever of satisfaction had relationship with the surface indentation (r = -.209, P = .033), skin with redness (r = -.202, P = .040), skin with uncomfortable (r = -.346, P < .0001), and plainness while removing (r = -.296, P = .002), skin with gummy residue after removing (r = -.259, P = .008), and deformation of sickness’ (r = -.201, P = .040). Conclusion: According to the results, it could provide the evidences to the improvement project of the innovation Catheter Fixation Strap to instead of micropore fixed, that could overcome its complication and thus enhance the quality of life in the comfort of the cases.
