本研究使用血管擴張劑前列腺素E1來做為陽萎病人之短期治療藥物。從1988年8月至1990年5月共有115位陽萎病人接受前列腺素E1(20μg) 陰莖海綿體注射，其中56位病人曾接受二次(31位)或二次以上(25位)之治療，我們以此56位病人做為研究對象。在客觀上，探討其第一次治療及其後續治療所達成陰莖勃起之角度，勃起之硬度及勃起之維持時間。其平均值分別為： 勃起角友為47.1° ± 21.6°及58.7° ± 22.6 (P＜0.01)，硬度為 2.79 ± 0.61及3.05 ± 0.67(P＜0.01)，維持時間為37.3 ± 17.0分鐘及39.0 ± 15.6分鐘(P＝0.07)。上述三項都有進步且能達到行房目的者有29人(51.8％)，雖有進步仍未能行房者有19人(33.9％)，退步者8人(14.3％)。利用性功能評量表調查治療前後之症狀，獲知大多數病(78.1％)在主觀上有症狀上之改善。其中有12位病人在治療前後進行Xe-133陰莖海綿體廓清試驗，發現有 10位病人在陰莖血流循環上獲得改善，一位不變，另一位退步。由以上主、客觀之結果分析，我們認為用前列腺素E1行陰莖海綿體注射做為陽萎病人之門診初步治療是值得推薦的方法。(J Urol R.O.C., 2:465-471, 1991)

This study used PGE1 as a short-course treatment for impotent men. One hundred and fifteen patients had re-ceived intracavernous injection of PGE1 (20μg) at our out-patient clinic from Aug. 1988 to May 1990, and 56 patients who had two or more injections were selected for this study. We recorded the response of first and last in-tracavernous injection, and the results was fol-lowing: eretion angle 47.1° ± 21.6° and 58.7° ± 22.6 (P＜0.01). rigidity 2.79 ± 0.61 and 3.05 ± 0.67(P＜0.01), maintenance 37.3 ± 17.0 min. and 39.0 ± 15.6 min. (p=0.07). Twenty nine patients (51.8%) improved in penile erec-tion angle, rigidity, maintenance, and could un-dergo intercourse. Nineteen patients (33.9%) had symptoms improved but were not able to intercourse. Eight patients (14.3%) had no im-provement. In this study, 12 patients had re-ceived X e-133 penile washout study, 10 pa-tients were improved in penile blood flow, one was the same, and one was worse. Sexual dys-function questionnaire was used to evaluate the condition of pre- and post- intracavernous injec-tion, and 32 patients (78.1%) personally felt symptoms improved. This study indicates that intracavernous in-jection with prostaglandin E1 may improve hemodynamics objectively and subjectively, it can be considered as a good choice for the ini-tial treatment for impotence.
