中文摘要

個案管理是一種以個案為中心,由各類醫療人員參與照護,達到預期的照護品質及控制醫療費用
的照護系統。為達到照護之連續性及即時性,個案管理系統資訊化為不可或缺之ㄧ環。建立個案
管理資料庫有助於提升資料管理效率、減少資料的重複性、提高資料安全性與保密性等,並且能
夠提供自動提示的功能及方便資料統計分析等。資料庫建置前必須考量資料庫要涵蓋的功能,充
分與資訊人員溝通合作,建置完成後可運用於個案管理過程之各個階段,協助個案篩選、連結個
案照護之相關資訊、作為個案管理照護紀錄、各項指標監測、保險申報參考、成本分析及研究
等,結合強大的資訊自動化功能不但能確保資料正確性與即時性,更能提高個案管理之服務品質
與效率。

英文摘要

Case management is a case-centered care model that leverages multi-disciplinary teamwork to pro-vide quality and cost-effective patient care. To ensure care continuity and timeliness, it is necessary
to automate case management systems. A case management database is essential to system effciency; helping avoid double data entries and ensuring data confdentiality and system security. The database
can also support data analysis and programming designed to generate useful statistics and automatic reminders. Identifying critical database functions is the frst step in database development. Once a database has been established, it can be applied to functions throughout the case management process, including case screening, care related data retrieval, patient record storage, quality monitoring, insur-
ance reimbursement and research. An effective information automation system will ensure the accu-racy and timeliness of information as well as enhance case management quality and effciency.
