Objective: Carotid artery stenosis (CAS) is an important risk factor for cerebral infarction. However, the effects of CAS on neuropsychological functions have been controversial. Furthermore, whether CAS ipsilateral to the cerebral hemisphere can result in different patterns of cognitive impairments has been increasingly emphasized. The purpose of this study intends to investigate the cognitive sequelae and lateralized effects of CAS.
Method: One-hundred-and-two patients with CAS were recruited from the Department of Neurology of Chang Gung Memorial Hospital (Lin-Kou). The patients were divided into three groups: the left CAS group (n = 36), the right CAS group (n = 34), the bilateral CAS group (n = 32) base on the results of angiography. Another 33 voltunees were recruited by advertisement as the control group. All participants underwent a dementia screening procedure and a series of neuropsychological assessment.
Results: The results showed that compared to the control group, CAS might produce adverse effects on general intelligence, memory, manual dexterity and executive functions. In addition, the left CAS group performed more poorly than the other groups on the verbal learning test and the manual dexterity test for the right hand, both of which are more related to the left hemispheric functions. In the case of the right CAS group, the participants' scores for the Design Fluency Test and a manual dexterity test for the left hand was significantly inferior to those of the other groups. The bilateral CAS group appeared to achieve the lowest scores for a non-verbal intelligence test, visual memory test and category fluency test.
Conclusion: These results suggest that cognitive impairments can be brought about by CAS. Additionally, the profile of cognitive deficits appears to be in line with the sidedness of CAS. This study demonstrated that the lateralized effects of carotid artery stenosis on cognitive functions may merit further investigations.
