Objective: The goal of this case study was to have an exploration on the role of social and physical activity in the treatment effect of CBT with a MDD patient.
Method: The patient is a 60 years-old retired female, married with two child. Her depressive symptoms (including depressive mood, insomnia) began from 1995. At that time, their family's land was seized as a result of her husband being someone's bondsman, which also led them to owe a debt of over 10 million. In the beginning, she did not go for treatment, until year 2002, she started to visit psychiatric clinic and take medication. After the debt was solved, her mood improved and she decided to stop taken medication. However, her depression relapsed, when she known that one of their land might be levied by government for the need of county development in 2005. In addition, the diagnosis of MDD had made her feel very shame as she could not accept any defect occurred in her life, she usually performed very well throughout most of her life stage. Besides, she worried her MDD would leave a stigma on her family, which might affect her second daughter's wedding progression. In 2010, as her depressive mood and suicide ideation had become more significant, she therefore was hospitalized. After discharge, she was then referred for psychotherapy. Twenty-six sessions were conducted once a week by a clinical psychologist from July, 2010 to January, 2011. In the progression of therapy, the focus was put on changing patient's pattern of thinking and encouraging her to get more involvement in activities. By the end of psychotherapy, her emotion became more stable, and the score of BDI-II and BAI had dropped significantly from pre- to post-treatment
Conclusion: Patient's depressive mood might result from both negative pattern of thinking and loss of her life focus. Accordingly, efforts was put on changing her pattern of thought, and encouraging her to get more involvement in all kinds of activities, as long as the activity required a higher energy level than bed rest. The purpose of having her to involve in any activities except for bed rest, was hoped to assist her in finding out a new life focus. In fact, at the end of the therapy, this study found several improvements were achieved by patient, such as more positive thinking, less catastrophic thoughts, less depression. Further, it found the enhancement in activity participation actually helped patient to broke her negative thoughts (e.g, others would look down on her because of MDD), reduced depressive mood and gaining a new life focus.
