Objective: This 73 year-old married male, C.F., was suffered from hypoxic encephalopathy following an acute onset of myocardial infarction. After hospitalization for 17 days, he was transfer to a nursing home. In three more month period of living in the nursing home, C.F. performed with impaired cognitive function, day-night disturbance, and dependent ADLs (activities of daily living). He was then taken to the NTUH Clinic of Physical Medicine & Rehabilitation, and the doctor referred to clinical psychologist for neuropsychological rehabilitation.
Method: In this case, neuropsychological-orientated rehabilitation was implemented, and therapeutic sessions were carried out once a week that continued for 6 months. Evaluation in the measures of neuropsychological test, adaptive behavior scale, and other ecological observation from families was included in this single-subject report. Discussions were probing into some facilitative factors for better treatment outcome.
Results: After 26 sessions within the 6-month rehabilitation, C.F. had significant improvement of cognitive functions and adaptive behaviors in personal care, domestic, community and social domains. Enhancement of self-awareness and confidence was also observed. Factors emphasized for facilitating community re-entry were 1) clear and definite rehabilitation goals, 2) structurized and supportive environment in family, 3) certainly implement of home program, and 4) raised active participation of patient himself.
Conclusion: Nursing home is a considered placement for aged people with impaired self-care abilities and cognitive functions in postacute stage of diseases. However, neuropsychological intervention with community-based rehabilitation programs could further facilitate functional independence and psychological well-being in patient care.
